At first, two workers from a subcontractor company who were cleaning the area fainted after breathing in gas. They were utilizing a cleaning vehicle with a hose to clean near the pipes when the accident happened. Officials believe the cause was carbon monoxide poisoning. Carbon monoxide has a higher affinity for hemoglobin in red blood cells compared to oxygen, resulting in a lack of oxygen and suffocation when inhaled in large amounts.

A POSCO employee, Mr. A (in his 40s), who found the victims, informed the company's internal fire department about the incident and subsequently became unconscious. Three firefighters who arrived at the location tried to perform rescue actions, including CPR, but later experienced dizziness and breathing difficulties and were taken to the hospital. As of 10 p.m., Mr. A and the two subcontractor workers were still unconscious. A police representative said, "They are on ventilators and in a critical state." The three firefighters were reported to be in stable condition.

Authorities from the police and fire departments are looking into the chance that employees inhaled carbon monoxide resulting from external damage or pipe degradation while cleaning. They will check if workers used protective gear to avoid exposure to dangerous gases and if POSCO provided a secure working environment. The Ministry of Employment and Labor has advised a "work halt" at the affected site.

The Pohang Steelworks has experienced three safety incidents this year.
